Our application process is easy and will move at a pace that is right for you and your family. You’ll be surprised how individualized foster care can be.

Below, you will find five documents that will begin our process in certifying you as an Oklahoma foster parent.

Foster Care Application

The Foster Care Application has + and - so that you can add to or take away from the areas that pertain to your household.


Request for Background Check

The Request for Background Check form should be filled out for each adult (ages 18 and up) in the home. By filling out this form, you will be giving HSRS permission to run the proper background checks for your family.


Driver Record Request

The DPS department of public safety (Driver Record Request) form should be filled out for each adult (ages 18 and up) in the household. This gives HSBR permission to obtain a driving record for each adult in the home that might be transporting foster children. Please include all adults over the age of 18 in the household.


Adult in Household Application

The Adult in Household Application should be filled out for any adult over the age of 18 who lives in the home and is not applying to be a foster parent.



The JOLTS Form should be filled out for every person in the household from the ages of 13 to 17. These must be filled out by an adult, currently residing in the household, over the age of 18 years old.
